Health Care Coordinator-F/T -Clinics - Northern Montana Clinics
The Health Care Coordinator-Coach shall assist in the overall management of Northern Montana Hospital (NMH) patients, particularly those with chronic illnesses, as well as assist the Chronic Care Navigator, and clinical team in identifying appropriate patients with chronic conditions for care management. He/She is responsible for performing population health care management and patient-self management support and clinical documentation functions. The Health Care Coordinator-Coach shall also provide support for patient and family centered continuity of care services for patients undergoing transition of care; as well as assist with providing patient and families with optimal linkage to community resources. He/She shall work collaboratively to improve quality of patient care through facilitating the efficient use of resources, and acts as an advocate for individual’s health care needs. Other duties include but are not limited to, individual, and group based education, counseling, and self-management goal setting, monitoring of patients’ plans, and collaborating with providers, patients, and families for coordinator of services.
Degree in health education, nursing, nutrition, or certification as Registered Medical Assistant , or three (3) years clinical experience in a primary care setting, or equivalent education and/or experience.
Provide proof of up to date training and certification in Healthcare Provider Basic Life Support issued by the American Heart Association, American Red Cross, or American Health & Safety Institute. Must obtain within three (3) months of hire, if license is not current.
Ability to obtain certification through the National Society of health Coaches.
Valid driver’s license with acceptable driving record for insurance purposes.
Must have excellent communication, and interpersonal skills, with the ability to work independently, as well as part of a team, and collaborate with co-workers, supervisor, providers, patients, and the public.
Must be able to maintain patient, team members, and employer confidentiality. Must strictly comply with all HIPAA regulations.
Must exercise clinical judgement in interactions with physicians, payers, patients, and components.
Must demonstrate strong organization and time management skills, as evidence by a capacity to priority multiple tasks and role components.
Must have proven strong analytical and data management skills.
Must recognize and abide by scope of practice boundaries, as well as be comfortable seeking direction, and assistance from appropriate resources.
Ability to proficiently use a computer and electronic medical records.
Must remain flexible, and be able to adapt easily to changing conditions and work responsibility, as well as be able to complete assigned tasks under stressful conditions.

Salary is based on certification, and commensurate with up to 10 years of experience.
